This form is to be utilized on behalf of all OMHA players wishing to attend “AAA Zone” tryout/training camps of hockey teams/associations within the OMHA and ALLIANCE. PTS forms will be emailed to you in March or Early April after the OMHA releases the updated form for the 2024-2025 Season.
