This form is to be used by OMHA players, U12 and up, wishing to attend tryouts for an OMHA AA Team at another association. You may only choose one AA centre.
You will receive an email in April when your NRP request has been sent into the OMHA. There is no form to submit to the Association you are trying out for, it is all done online now.
