Permission to Skate & NRP Forms
Permission to Skate (PTS) and NRP forms for AAA and AA tryouts will be available late August / Early September. More information will be posted on the website when available.
Junior PTS forms for Jr. Hockey Clubs located within the Ontario Hockey Federation (OHF) may now be requested by eligible players. Please contact the HMHA Director of Hockey Operations to request a Junior PTS form at [email protected]
