This form is used to request an approval for team fundraisers.
Once submitted it is reviewed by the Fundraising and Sponsorship Director.
Once reviewed the Sponsorship Director will contact you to either approve, reject or ask for more information.
You acknowledge that you will follow the fundraising guidelines stipulated in the HMHA Rules of Operations.
