Permission to Skate (PTS) & Non-Resident Passport (NRP) Forms

Permission to Skate (PTS) and Non-Resident Passport (NRP) requests are now being processed.

All requests for PTS's and NRP's must be made using the online form.  

Please complete the Online PTS and NRP Request Form using this link.   

All players must be pre-registered with HMHA in order to receive a PTS or NRP form.  Forms will not be issued to non-registered players.  For clarification, players must only be registered with HMHA.  No payments are required until the players status at AAA or AA tryouts is known.

PTS forms will be emailed to you once the information submitted and HMHA registration has been verified.  You will need to print the form and present it at the first AAA tryout. 

NRP forms must be picked up in person and presented at the first AA tryout.  Pickup date, time and location will be posted when available.
