Permission to Skate (PTS) & Non-Resident Passport (NRP) Forms

Permission to Skate (PTS) and Non-Resident Passport (NRP) requests are now being processed.  

All requests for PTS's and NRP's must be made using the online forms.

Please complete the Online PTS and NRP Request Form using this link. 

PTS forms will be emailed to you by April 8th. You will need to print the form and present it at the first AAA tryout. 


NRP forms must be picked up April 15th between 6:30pm and 8:30pm in the HMHA office at Hespeler Arena and presented at the first AA tryout.
