Exhibition Game Request Form

Use this form to request an approval for an exhibition game. Once submitted it is reviewed by the HMHA Ice Scheduler. If approved it is sent onto the Director of Hockey Operations for Travel Permit. Associated costs will be billed by the Treasurer and the team will be invoiced if applicable.
