Use this form to request approval from the HMHA to attend a tournament. You can also request that HMHA pays the tournament fee. If HMHA pays the fees then the team will be billed by HMHA later in the season.

Once submitted the request is reviewed by the Ice Scheduler and Treasurer.

If approved it is sent onto the Director of Hockey Operations for Travel Permit.

Associated costs will be billed by the Treasurer and the team will be invoiced if applicable.
