IT IS MANDATORY PRIOR TO ROSTERING THE PLAYERS ON A TEAM THAT EACH PLAYER'S PARENTS/GUARDIAN PROVIDES A COMPLETED ACKNOWLEDGEMENT FORM.

This is a mandatory requirement brought into law by the Ontario Government. Without signed Acknowledgement Forms on file with the HMHA, no registered player will be allowed to play or be rostered with the OMHA on any HMHA Team.

Review the age appropriate resources that can be found under the Rowan's Law Tab on the HMHA Website with your child then complete the OHF Rowan's Law Acknowledgement Form.
